You may only register at John Hancock Charter School after we have notified you of a vacancy that can be filled by your child.  To register you need the following:

(   )   Emergency Information Form - Click to download.

(   )   Immunization Form - Click to download.

(   )   A copy of the student's birth certificate.

If you are not able to download the blank forms you can pick a copy up from the school.

Utah State Law requires that each child be registered at only one school at a time. If you register with us you must call the school you were registered at and tell them you are registered with us. If you choose to leave JHCS after registering you need to notify us.
