Enrollment Information
Kindergarten 2008-2009 Intent to Enroll
The Enrollment Process:
Fill out an intent to enroll form and deliver it to the address listed below.
OR fill out the online intent to enroll. You will have to fill out your family information and then add your children.
If
enrollment exceeds capacity during the open enrollment period, then the
classes that exceed capacity will go to a [[lottery]] as required by
the law.
After
the open enrollment period has ended you will be notified if your
child(ren) may be enrolled. You will then be given time to complete the
enrollment process by [[registering]] your child(ren).
If you fail to register your child(ren) during the time alloted you will forfeit their positions in the school.
While
you may fill out an intent to enroll form for more than one school, we
do require that you only be registered at one school at a time.
How to fill out an Intent to Enroll:
Turn
your completed intent to enroll form to Ms. Mitchell between 8:30 A.M.
and 12:30 P.M. at 125 N. 100 E., Pleasant Grove, UT 84062. Her phone
number is 796-5646. Her email is office@johnhancockcs.org. She can provide you with a blank form or you may download it here.
